Leading the way forward in dementia and challenging behaviour care
 
The Coniston centre comprises of two units Kendal and Keswick within Lakeview Care Home. The two units unit form part of its specialist care centre with a total of 29 Challenging Behaviour for dementia beds.
 
The Coniston Centre provides specialist nursing care for people with cognitive impairment combined with complex needs and/or challenging behaviours.

Our aim is to ensure that service users can lead as normal a life as possible whilst minimising the effects of challenging behaviour and maintaining a focus on each individual’s abilities rather than the deficits associated with dementia. 

Our model of care is based on a person centred approach which begins with a full evaluation of the individual’s care needs so that we can deliver care pathways which reduce the impact of cognitive and behavioural disturbances. We aim to deliver a safe, structured, respectful and caring environment, staffed by experienced professionals working in conjunction with an extensive multi disciplinary team.

Service users will be given every opportunity to maintain and develop social, emotional and
psycho educational skills, including communication and independent living skills, as set out in their person centred plan. Many social and recreational activities are organised and a full activities programme is displayed on each unit.

Acceptance Criteria 

The dementia and challenging behaviour service accepts adults over the age of 55, who are suffering from dementia and have complex needs including early onset dementia and acquired static or progressive dementias with challenging behaviour

Admissions to the unit are based on a full needs assessment and liaison with the appropriate authorities or representatives.  A place will only be offered if the needs assessment indicates that the unit can meet the needs of those assessed.
